Aanvragen patiëntenomgeving / register patient portal

Dit is alleen beschikbaar als u bij de praktijk bent ingeschreven als patiënt. Wilt u zich inschrijven als patient? Klik hier.
U kunt als volgt een account voor de patiëntenomgeving aanvragen:
Optie 1 (aanbevolen): Op uw telefoon:
Stap 1: Download de app op uw mobiele telefoon:
Stap 2: Na het downloaden van de app: volg verdere instructies in de app
Optie 2: Via de website:
- Ga naar https://reguliersgracht.uwzorgonline.nl/inloggen-en-registratie/app/
- Klik ‘maak een nieuw account aan’
- Volg verdere instructies op de website
Nadat u het account heeft aangevraagd, koppelen wij zo spoedig mogelijk uw online-account aan uw medische gegevens in ons zorginformatiesysteem. Zodra het gekoppeld is, krijgt u een bericht in uw e-mail. Vanaf dan kunt u gebruikmaken van de patiëntenomgeving, inclusief e-consults = beveiligde e-mail.
Lukt het niet om een account aan te maken?
Stel uw vraag dan via www.uwzorgonline.nl/vraag of bel de helpdesk Digitale Zorg op 085 – 13 04 575.
Ze zijn op werkdagen van 8.00 – 18.00 bereikbaar. Mailen kan ook: info@helpdeskdigitalezorg.nl

Do you not have an account for the patient portal app yet?
This is only accessible if you are registered as a patient at Huisartsenpraktijk Reguliersgracht. Would you like to register as new patient? Click here.
Registration for an account for the patient portal is for English users not available through desktop, but only through app. Good news: the app is easier to use. You can apply for an account at the patient portal. Please take the following steps:
Step 1: Download the app Uw Zorg Online
- iPhone: click here
- Android: click here
Step 2: After downloading the app, choose ‘register’
step 3: Follow further instructions in the app
Once you have applied for the account we will link it as soon as possible to your personal medical file in our EHR (Electronic Health Record). You will get a notification in your email when it is done. From then on you can use the patient portal including e-consults.
